Fallout's costume designer explains the creation of his grisly characters

Talk to Polygon, associate costume designer Amy Westcot explained how the costume team designed different “ghoul” stages, of which there are “so many different degrees.” In the games, ghouls can exist as an average person with the right medications, otherwise they become feral and attack virtually anything that moves. They're this world's zombie equivalent, just in two different flavors, one of which is “kind of on the verge of extinction” and considerably more deadly. But watching ghouls go from normal to Feral is not a pleasant experience, one that the show's creators clearly wanted to make as clear as possible.

For the series, keeping the ghouls in a mostly human state was “super important”, especially regarding their humanity. Westcott revealed that the costume department worked hard with the textile team to ensure they were age-appropriate while also garnering sympathy from the audience. “They were people once, and it was important to remember that, even the wild ghouls,” she noted. “They were supposed to be in rags, but… there's still a trace that they were once a person, and I think we all sympathize with them as well.”

In the case of THE Ghoul, Westcott said he couldn't help but be the least grotesque of the group we see. Throughout the season, we see him swallowing the aforementioned drugs, which allow him to “always (have) his senses” and stand out from other ghouls. He's not entirely one way or the other, but it was vital that he looked like an ordinary (albeit irradiated) person while being “on a different level.” scene macabre, (he) just couldn't be Feral. With a second season on the horizon, it might end up moving closer to Feral territory.
